New Patient Registration

Please bring along your medical card if you have one. You will be asked to complete our New Patient forms.

New practice area as of 3rd August 2016

On registering with us you will need to provide current proof (within the last 2 months) of your address*.

Accepted documents are: -

  • Mortgage/Bank Statement
  • Rental agreement
  • Utility bill
  • DWP documents

Any patient aged 16 or over is entitled to a general health check with a nurse when they register. Please ask the receptionist for an appointment.

*if you do not have the documents accepted please speak with reception.

Disabled Access

Disabled patients are always welcome at our surgery.

Change of Address, Name & Telephone Number

Please let us know as soon as possible of any changes to your name, address and telephone number.

If you move out of our practice area it will be necessary to register with another doctor.
