New Patient Registration
We welcome all new patients who live within Practice boundary. You will be asked to complete our New Patient forms. We also require proof of address and (photo ID for online access.)
On registering with us you will need to provide current proof (within the last 2 months) of your address*.

Accepted documents are: -
- Mortgage/Bank Statement
- Rental agreement
- Utility bill
- DWP documents
- Driving licence
Any patient on regular repeat medication will need to have a review with our clinical Pharmacist within 3 months of joining. This is so we can provide safe prescribing for all our patients. Please book an appointment with reception.
*if you do not have the documents accepted please speak with reception.
Disabled patients are always welcome at our surgery. All our consulting rooms are on the ground floor.
Change of Address, Name & Telephone Number
Please let us know as soon as possible of any changes to your name, address and telephone number.
If you move out of our practice area it will be necessary to register with another doctor.
